MO WORK PERMIT INSTRUCTIONS

1.  Go to the following Missouri Department of Labor and download the Work Permits - Entertainment Industry PDF, located at the bottom of the page.

2. Fill out the form with your information (see example below).

3. VERY IMPORTANT: Be sure to CHECK THE BOX “Yes”, to request that the hour limitations be waived.

4. You will need to provide a copy of the birth certificate or other form of proof of age. This can be a picture or scan of the document.

5. Email the department of labor the form and also attach the proof of age document. Request that the Missouri Department of Labor emails you a copy of the work permit.
